Frequently Asked Questions

Answers to the practical questions patients ask before their first visit.

Getting Started

Do I need a referral to be seen?

Most insurance plans don't require one, but every insurance plan is different. Some of our Medicaid patients, for example, need a referral before their visit, so it's worth double-checking with your specific plan before you book.

How do I book an appointment?

Book online, call 1-888-729-4999, or email info@clearchoicederm.com. Choose your preferred location, provider, and timing.

What should I bring to my first appointment?

Photo ID, insurance card, current medication/allergy info, relevant records from previous providers, and (if applicable) photos of your skin condition and the actual products you're using.

Can I fill out my paperwork before I arrive?

Yes. Download and complete the new patient forms ahead of time to speed up check-in.

How long does a first visit take?

Most first visits take about 20 to 30 minutes from check-in through check-out, though this varies by the reason for your visit.

Do you offer virtual visits?

Not currently. All visits are in person at one of our clinics.

Do you take walk-ins?

No, we don’t take walk-ins.

Insurance & Payment

Do you accept my insurance?

We accept a wide range of plans. See the full accepted insurance list on our Insurance & Billing page. That said, every insurance plan is different, and coverage/requirements (like referrals) can vary even within the same carrier, so it's worth confirming your specific plan's rules before your visit.

What if I don't have insurance?

We offer competitive self-pay pricing for uninsured patients, patients whose plan isn’t accepted, and out-of-state visitors.

Will you bill my insurance for me?

Yes. As a courtesy, we submit claims to your insurance carrier on your behalf, including secondary and tertiary claims where applicable.

How do I pay my bill?

Online through the patient portal, by phone at 1-888-729-4999, or by mail. See Insurance & Billing for details.

What is a Good Faith Estimate, and how much does it cost?

A Good Faith Estimate is required for self-pay and uninsured patients under the No Surprises Act. It costs $200 for new patients and $150 for current patients.

During & After Your Visit

How do I message my provider or see my results?

Through our patient portal (EMA). You’ll get an invite after your first visit.

How do I reschedule or cancel?

Through the patient portal, or by calling your clinic directly.

Where can I find pre- and post-procedure instructions?

See our Pre- & Post-Procedure Instructions page for guides specific to your treatment, from injectables to laser to Mohs-adjacent skin procedures.

How do I request a copy of my medical records?

Submit a Medical Records Request form, available as a PDF through the patient portal page.

Is my information kept private?

Yes. See our HIPAA Notice of Privacy Practices (available in English and Spanish) for full details on how your information is protected and used.
